How can you access services?

  1. When registering for a program, indicate in the box on the registration form titled: "Accommodations" or "Special Needs" that you or your child may need support while participating in an MSCR program. The registration will be routed to Inclusion Services and then a staff will contact you for further information.

  2. To expedite the process, please call or e-mail the Therapeutic Recreation Specialist assigned to your area of Madison and indicate that you or your child has registered and is requesting accommodation. If you are uncertain how to proceed, call 204-3025 for assistance.

  3. A "Participant Profile Form", can be downloaded (Click here) or can be mailed to you. Please provide specific information on the needs of the participant and return the form. This helps to determine necessary accommodations.
  4. The Inclusion Services Team will review the form and work with you to determine reasonable accommodations. Meetings with the program staff my be necessary to implement accommodations. If additional staff is necessary for successful participation, it may require more time to coordinate services. Please remember MSCR's programs are voluntary in nature. Although the participant's growth and development are priority, therapeutic or clinical levels of intervention are not available. All participants must adhere to the MSCR Behavior Policies.

  5. Please allow at least two weeks for services to be arranged, otherwise services may be delayed.
